Medical Provider Contact Information

A list of provider contact information is below.

Check your carrier’s website for the most up-to-date information about what to do if your employees are experiencing symptoms and how they can access care. Carriers have or will be sending communications out to company administrators, please watch for these updates.

It’s important to direct your employees to call your provider ahead of time rather than going directly to a facility or making an appointment online. Calling ahead helps the provider direct your staff to the most appropriate care, and take precautions to protect other members, patients, and employees.

Nurse hotlines and virtual visits are also great resources for medical advice and access to care. 

Member ID cards will have contact and website information.

Kaiser:
